Electronic Rx - problems being encountered
Please share your experiences with e-presribing and associated problems

Our problems that we encounter on a daily basis have to do with the fact that we are located in Childrens Hospital of Wisconsin. Most of our rxs that are electronically generated are for unique needs and have to do with rare medications or compounded rxs. Whenever we receive a compounded rx, it is almost always incorrect. We have to clarify EVERY time. This is concerning becuase WE know that there is an error, but outside pharmacies that rarely receive compounded rxs would have no clue how to fill the rx and a mistake could easily be made. The other situation that I see regularly is where the prescriber has picked the wrong medication off of the list or they cant find the medication that they want, so they just choose the one that is the closest. For instance, nebulized albuterol. I called the prescriber after receiving the rx and they said they just wanted the premixed albuterol nebs. I went through the list over the phone for 5 minutes and we couldnt figure out which one to use. So in other words, the database of "medication lists" is not complete or at least very confusing to figure out what they really want.
