Focus Grants as referenced by the Community Pharmacy Foundation is an exciting opportunity, primarily for colleges of pharmacy and pharmacy associations to work with CPF in designing a major study/project in collaboration with community pharmacies and pharmacists.
Focus grants to date have involved matching funds and/or in-kind services whereby a college of pharmacy or a state pharmacy association has partnered with a local funding organization (i.e. Blue Cross Blue Shield Foundation) or a local health plan or PBM to implement Medication Therapy Management (MTM) services through a network of participating community pharmacies. One such network involves a synchronized group of community pharmacists, some retired and others who perform special patient care services on their days off … in the patient homes, by phone or at a cooperating pharmacy in the area.
Focus grants provide the opportunity for more sizable grant awards in order to implement innovative programs by a network of community pharmacies and pharmacists, for which endeavor includes a strong assessment and evaluation component to measure and validate patient outcomes.
