Objectives
Currently, Kroger Pharmacy in Columbus, Ohio participates in the 340B drug pricing program and provides comprehensive pharmacy services for the patients of East Central Health Center ECHC, a federally qualified community health center FQHC. As an additional service for ECHC and its patients, Kroger pharmacists will be performing medication reconciliation services for ECHC patients. During the patients medical visit with a ECHC physician, the physician will refer the patient to Kroger Pharmacy for a medication reconciliation session. During the session, Kroger pharmacists will collect the patients medication history, ensuring that the medications and doses are appropriate, and will document the patients personal medication record and medication action plan to be provided to both ECHC and the patient. Medication reconciliation at the pharmacy level will allow pharmacists to focus strictly on medication therapy management and physicians to address the patients medical issues during the patients initial visit and will allow the patients to become acquainted with the contracted pharmacy, where they may choose to fill their medications.
