Objectives
Practice transformation, through the actions of Community Pharmacy Enhanced Services Networks USA (CPESN), supports community pharmacy partnership while advancing patient care programs. Currently there are >75 state and national contracts supported within CPESN and its state chapters. Outlining best practices for implementation and evaluation of payor programs partnering with clinically integrated networks of community pharmacies is needed for scalability and sustainability. CPESN USA has established Data Sharing Agreement (DSA) pathways to facilitate research, program evaluation and data analysis with eCare plan submissions on interventional and other available data. However, access to most outcomes-based data needed to perform complete program evaluations requires greater payer collaboration. The objective of this grant proposal is to outline national best practices for partnerships between payors and clinically integrated networks of community pharmacies for patient care program implementation and evaluation. Through this work proven, replicable, and scalable approaches (e.g., outcomes, sharing data) will be developed and disseminated.
