Objectives

The goal of this funding request is to offer scholarships to CPESN Mississippi pharmacies to train one pharmacy technician on staff as a Community Health Worker (CHW). This project is in partnership with the Mississippi State Department of Health and will generate a statewide SDOH resource library, with the ultimate goal of creating a patient-facing, interactive resource map for the state of Mississippi. As a part of the CHW training, each technician will generate a list of SDOH resources within a certain milage of their pharmacy and report those resources to CPESN Health Equity to build a statewide SDOH resource library. The technicians will also track any patient referrals made to those resources as a result of their CHW training. Our overall goal is to quantify the direct impact to patients from having a CHW on staff at local community pharmacies, as well as generate a resource library for the state of Mississippi.
