CPF REPLICATION AWARD

This Grant Award is being offered to the community of pharmacy in partial celebration and recognition of the 10th anniversary creation of the Community Pharmacy Foundation by awarding $10,000 to the Principal Investigator implementing a “Replication Grant.”
 
The primary purpose of this special Grant Award by the Community Pharmacy Foundation is to take “lessons learned” from a completed CPF Grant and implement/replicate it in another community pharmacy.
 
This is a “TO DO” Grant Award … replicating a project already implemented through a previous grant of the Community Pharmacy Foundation in another community pharmacy that is not currently offering such service(s).
 
This Grant Award replicates a completed CPF Grant into another community pharmacy, and  …
 
1.      Completed project must include a “How I Did It” report, in addition to other customary aspects of a completed CPF Grant Award
 
2.      It must include collaboration with the Principal Investigator of the CPF Completed Grant
 
3.      It is recommended that collaboration exists with an appropriate person of a college of pharmacy of the awardees choice
 
4.      It must include a submission upon completion of the project for presentation or poster at the next APhA and NCPA meetings.
