CPF FUNDED GRANT DETAILS

Through this grant Apple Discount Drugs wishes to start a center for excellence in a community with an aim to provide clinical pharmacy services to underserved patients living on the rural Eastern Shore of Maryland. Apple has approached a local physician, who has worked in an advisory role with the pharmacy, and is willing to provide medical oversight to the center for excellence and function as the sites medical director. Apple is attempting to build a business model that is financially self-sustaining and can be replicated at other sites. Apple wishes to build through their center of excellence, a patient centered medical home, providing medication therapy management and disease state management. Apples primary focus for disease state management is their high risk diabetic population. The surrounding tri-county area around the pharmacy shows a diabetes incidence rate of 14.3% compared to a incidence of 8.3% statewide. Our focus will be on educating the individuals on self management skills with the ultimate goal of patient ownership of their disease, improving the patients quality of life, preventing complications and reducing total health care costs.
